Community Health Worker: Now Serving YOU!

Thanks to a recent grant from the University of Chicago Medicine Liaisons in Care (LINC) program, TRC is excited to introduce a Community Health Worker to the staff. The goal of this new position is to reduce neighborhood health disparities in South Chicago through outreach. We’re excited to work with our new community health worker Brittany in alignment with our goal of serving underserved communities by listening to their needs.  

I want the community to see me as a liaison for any of their needs ranging from health services to utility assistance. I provide education and connections to health and social resources. I want the people of this community to take advantage of the services that I offer.

  • Navigating health care and social service systems  

  • Referrals to health care & social service providers

  • Completing applications to access health benefits 

  • Helping you understand your health condition(s) and develop strategies to improve your health and wellbeing 

  • Referrals to free & low-cost legal services

  • Geriatric care services 

  • Utility bill assistance programs 

  • Transportation to and from medical appointments 

  • General health information and resources
